Learning from incidents, or LFI, is just one of many activities in managing safety in an organisation. LFI is defined as “a process through which employees and the organisation as a whole seek to understand any negative safety events that have taken place to prevent similar future events.” The outcome usually should lead to changes in behaviour or technical process in any organisation if all stages of LFI are properly implemented.

In terms of barrier-based approaches to risk and incident management, LFI can be achieved through linking proactive risk models (the Bowtie) to data from incident analysis tools such as Tripod Beta, BSCAT and Barrier Failure Analysis (BFA).  This enables improvements to the initial risk assessment to be made.

Steve Jewell, Offshore Well Abandonment Expert

Steve’s expertise is in all aspects of Well Construction (drilling, completions, workovers and abandonment) and Subsurface (geoscience, reservoir engineering, production engineering). He has spent over 10 years working on various decommissioning projects across the world focusing on the largest single element of cost: the plugging and abandonment of wells. This experience has covered actual field decommissioning projects, P&A planning, new technology, decommissioning deferment justifications, mature field asset trading and training for both engineers and non-technical staff.

Over the last 3 years Steve has developed and delivered two distinct well decommissioning courses which have been provided in the UK and Danish sectors of the North Sea for a variety of field operators, service companies and governmental organisations.

Steve is a UK Chartered Engineer (CEng) and a Fellow of the Institute of Materials, Mining and Minerals (FIMMM). He is also registered as a European Engineer (EurIng) and is a member of both the Institute of Directors (MIoD) and the international Society of Petroleum Engineers.
